Targeting doulas distracts from real issues in maternity care

The Doula Network Australia (DNA) has responded to the joint RANZCOG and ACM call to “end freebirth deaths,” urging authorities to focus on systemic reform rather than blaming non-clinical birth supporters.

“Targeting doulas is a distraction from the very real harm being caused by an under-resourced, over-medicalised maternity system,” said Samantha Gunn, President of DNA. “We share concern about preventable harm — but the solution lies in fixing the system, not vilifying those who support women within it.”

DNA emphasised that freebirthing is not illegal, and that every woman has the right to decide whether or not to involve clinicians in her care. Research from the World Health Organization and Australian Institute of Health and Welfare shows that continuity of midwifery care delivers better outcomes — yet limited access and previous birth trauma continue to drive some women to birth outside the system.

The network warns that over-regulation risks criminalising women’s choices and misrepresenting the non-clinical role doulas play. “Doulas provide emotional and informational support — they do not manage births,” Gunn said. “Real safety requires trauma-informed, respectful, evidence-based care that listens to women.”

DNA is calling for collaborative policy reform to expand access to quality care, strengthen midwifery continuity, and include consumer voices in maternity policy.
